ABSTRACT:
Nurse-led management of chronic disease has emerged as a crucial component of modern healthcare systems, particularly given the increasing burden of chronic conditions like diabetes, hypertension, and cardiovascular diseases. These conditions require continuous, multifaceted care that can often overwhelm traditional healthcare structures. Nurses, due to their comprehensive training and frequent, direct interaction with patients, are uniquely positioned to take on leadership roles in managing chronic diseases. Their ability to provide ongoing monitoring, offer personalized patient education, and ensure consistent follow-up care is central to improving disease management. By leading care teams, nurses facilitate coordinated interventions, ensuring that all aspects of a patient’s health are addressed, from medication management to lifestyle changes. Moreover, the nurse’s role in educating patients not only empowers individuals to take control of their health but also helps in building long-term compliance with treatment regimens, which is critical for disease control. This patient-centered approach fosters an environment of trust, where patients are more likely to engage in proactive health management. Additionally, nurses work collaboratively with other healthcare professionals, contributing to an interdisciplinary team that enhances patient outcomes through the pooling of expertise.
