Abstract
Chronic diseases, such as diabetes, cardiovascular disease, and chronic obstructive pulmonary disease, represent a significant burden on global healthcare systems. Nurses are increasingly recognized as vital agents in the management of these conditions. This comprehensive review examines the multifaceted role of nurses in chronic disease management, focusing on patient education, care coordination, evidence-based practice, and interdisciplinary collaboration. Through an analysis of recent literature and previous studies, this paper synthesizes current research findings to highlight effective nursing interventions and strategies that improve patient outcomes. The review also discusses challenges such as resource constraints, patient adherence issues, and the need for continued professional development.
